About Wells

Well Specifications Required for Permit

Provide the following information about your wells:

• All wells should be clearly labeled on the site map and the Well Owner's ID number must be included
• Drill hole diameter in inches
• Casing diameter in inches
• Surface seal depth in feet
• Maximum depth in feet
• Latitude and longitude (optional)
• Check to find out if you need to fill out a Site Hazard Form

For well destruction, you will also need:

• Original permit number issued for well construction (optional)
• State well number assigned to well constructed (optional)
• Original WCR number for the well construction (optional)
